Abstract
Guided Biofilm Therapy: prevention and maintenance on implant and non surgical treatment of peri-implantitis.
When we choose to replace a tooth with an implant we are deciding to replace a biological organ with a prosthesis which will function differently due to anatomy, histology and biology. The clinical biometric parameters that we will use to evaluate the health of the implant will also have to be evaluated differently. Non-surgical maintenance will be performed with different instruments. All of this is closely related to the 4 mm of soft tissue surrounding the implant abutment. This tissue must have phenotypic and biotypic characteristics such as to be able to survive over time without recession and such as to withstand the chronic inflammatory state which is always present at a marginal level. Which is why achieving adequate thickness and height of soft tissue with as much seal as possible is crucial.
The apico-coronal position of the implant will become fundamental in anticipating the re-establishment of the biological width, a connection that avoids microleakage will be essential to be able to position the implant under the bone crest, the platform switching will be mandatory to give space to the soft tissues and the position of the margin of the prosthetic restoration no further than 1-2mm from the gingival margin.
All this represents the primordial prevention that must be followed by the primary one, the possible intervention for the treatment of mucositis or peri-implantitis and the secondary prevention. The Guided Biofilm Therapy represents a standardized and effective non-surgical approach for the maintenance and for the step 2 of non-surgical therapy that can be associated with the use of topical antiseptics or can be a preparation for surgery. All these aspects will be addressed in the lecture trying to give clinical ideas on how to face the difficult task of prevention and treatment of biological complications.
